ABBEYVIEW SURGERY
NEWS – November 2016
ABBEYVIEW SURGERY IS NATIONAL FINALIST – TWICE!
We are extremely proud to have been selected by the National Association of Primary Care as a top three national
finalist in the category of Innovative Pathways.
We are also very proud to have been shortlisted for The General Practice Awards 2016 for Clinical Team of the
Year – Long Term Conditions category.
This is in recognition of the additional work which the Practice, with the valued support of our Patient
Participation Group, has undertaken to try and improve the care and support locally for patients with Dementia
and for Carers.
The work has included developing roles for a Dementia Lead and Carer Champion at the Practice, tailored Carer
and Dementia packs, Carers Corner in the waiting room, Dementia/Carer events, Dementia Friends training for
staff and community. We have liaised with local support agencies including Crowland 55+ Group to gain
information of local resources to support our patients and carers. We have also worked closely with Lincolnshire
Carers and Young Carers Partnership to develop the Lincolnshire Carers Quality Award. Abbeyview Surgery
being the first business of any kind in Lincolnshire to receive the award.
Overall winners will be announced on 19 October (NAPC) and 24 November (General Practice Awards).
