South x 11 x General CommentGeneral CommentCase 5092 (25-09-2018)Providers: Abbeyview Surgery, Lincolnshire County Council – Adult Social CarePatient had a “well-being” call at their home to give advice. Suggested several things which could help make life easier but all at a cost that they couldn’t afford. The Practice Care Co-ordinator arranged a shower seat, which was free. However, it was not fitted, advised a fitted seat would cost. Patient believes to get financial help they will have to go through lots of paperwork which they find difficult.No patient details to follow up HWL – asks how do we ensure that when equipment is issued the return on investment is assured by getting the equipment installed?Abbeyview Response – The Care Co-ordinator works from the Practice to support patients with frailty and other co-morbidities. Her role includes signposting services which may support patients to remain independent safely for longer, improving their quality of life. She does not provide any equipment directly and is therefore not involved with analysing return on investment – this enquiry therefore needs to be directed to the service providing the shower seat. If a patients informed the Care Co-ordinator of a need for support with paperwork, there are agencies that she is would be happy to signpost patients to, on request.

Extended Access Provision

We are pleased to be able to offer our patients access to routine appointments at evenings and weekends.

The new service is part of an extended access service which has been commissioned by NHS South Lincolnshire Clinical Commissioning Group.

Appointments are for pre-bookable non-urgent appointments and are available between 6.30pm and 8pm Monday to Friday, and 9am to 12pm on Saturdays and Sundays.  Appointments will also be available on bank holidays and across the Easter, Christmas and New Year periods

The service see practices across the area working closely together to provide convenient high quality care and may mean that you can take advantage of an appointment with a health professional from another local practice.
