Suggested several things which could help make life easier but all at a cost that they couldn’t afford. The Practice Care Co-ordinator arranged a shower seat, which was free. However, it was not fitted, advised a fitted seat would cost. Patient believes to get financial help they will have to go through lots of paperwork which they find difficult. No patient details to follow up HWL – asks how do we ensure that when equipment is issued the return on investment is assured by getting the equipment installed?Abbeyview Response – The Care Co-ordinator works from the Practice to support patients with frailty and other co-morbidities. Her role includes signposting services which may support patients to remain independent safely for longer, improving their quality of life. She does not provide any equipment directly and is therefore not involved with analysing return on investment – this enquiry therefore needs to be directed to the service providing the shower seat. If a patients informed the Care Co-ordinator of a need for support with paperwork, there are agencies that she is would be happy to signpost patients to, on request.
